Move ph and dh to Helm
-- Helm charts for ph and dh
-- New container to do Consul k-v storage and service reg
-- Bootstrap container no longer deploys ph and dh
-- Healthcheck looks for ph and dh as Helm deployments
-- Updated/added licensing information
